Application for Services
We value ALL athletes in our area and their journey to achieve their goals. The application process is confidential and includes a short application form and requires only a copy of the first page of the current year's tax form for financial assistance verification. These documents are securely stored at Change the Game for the life of the athlete's tenure in our programs. Financial assistance is determined by the size of the household and current income on a sliding fee scale. All fees are due once financial assistance is determined and at the time of service. Applications must be renewed yearly. If your situation should change during the year you can resubmit to the Foundation for further assistance. Financial assistance is only good for services provided at Change the Game and as listed below.
